A reverse Abbe flap (Estlander-Abbe flap) is the exact same flap only taken from the upper lip to reconstruct lower lip defects not involving the commissure. We will use the technique for the traditional Abbe flap for this chapter, but recognize the reverse flap is the same technique based on the superior labial artery.

1. The flap is based on the commissure and is outlined in the lower lip area (Fig. 6.24a). It is rotated 180° and sutured into the wedge-shaped defect in the upper lip (Fig. 6.24b, c).

Balance between the lips is preserved because the resulting length of the donor lip is proportional to the length of the recipient lip.

Abbe-Estlander flap was used in each of the 10 patients to reconstruct the various lip defects. Various sites of the lip defects are shown in [Table/Fig-2] that was reconstructed by using Abbe-Estlander flap. 5. After 4 to 6 weeks the pedicle is divided and the lips are released. Nearly 100 years later, in 1872, Estlander described a single-stage cross-lip flap used to reconstruct a lateral defect that involved the oral commissure that was similar to that originally described by Hierzel (Tommaso 2009). The flap that Estlander described involved the transfer of a full-thickness flap that used a point of rotation at the Conclusions: Combined bilateral Karapandzic and Abbe/Estlander/Stein flaps can produce excellent functional and aesthetic outcomes in near total and total lower lip reconstructions and should be considered a reliable reconstructive option in patients with more then 70% of lower lip loss. The Abbe and Estlander flaps are useful in the reconstruction of large, full-thickness upper or lower lip defects. Such defects may be the result of trauma, excision of neoplasm or other.
